Supports Coordination - Developmental DisabilitiesABI Supports Coordination services are committed to supporting individuals and their families within the home and community. We recognize that individual needs and their families can change overtime, therefore, we take an individualized and person-centered approach to our efforts and support.
Supports Coordination is a service that provides the consumer/participant with a trained staff person who will assist them with obtaining services in the home and community setting. A Supports Coordinator will meet with the consumer/participant in their home to discuss services. The Supports Coordinator will assist with linking the consumer/participant to individualized services that best suit the individual needs of the consumer/participant. The Supports Coordinator will also assist with identifying options for all supports in your plan, including informal, mainstream and community needs ABI provides the following Supports Coordination Services to individuals with developmental disabilities and their families to help individuals achieve their goals:
