Supports Coordination - Developmental Disabilities

ABI Supports Coordination services are committed to supporting individuals and their families within the home and community. We recognize that individual needs and their families can change overtime, therefore, we take an individualized and person-centered approach to our efforts and support.

Supports Coordination is a service that provides the consumer/participant with a trained staff person who will assist them with obtaining services in the home and community setting.

A Supports Coordinator will meet with the consumer/participant in their home to discuss services.

The Supports Coordinator will assist with linking the consumer/participant to individualized services that best suit the individual needs of the consumer/participant.

The Supports Coordinator will also assist with identifying options for all supports in your plan, including informal, mainstream and community needs

ABI provides the following Supports Coordination Services to individuals with developmental disabilities and their families to help individuals achieve their goals:
  • Development of trusting relationships that are grounded in mutual respect and dignity
  • Creation and maintenance of an individualized support plan that identifies meaningful and measurable outcomes
  • Assessing the need for services and funding
  • Accessing supports in the community
  • Locating service providers and agencies
  • Coordination of services and team members
  • Monitoring to ensure that individuals served are receiving services and supports as outlined in their individualized plan
  • Monitoring to ensure the health, safety, and well-being of individuals served
​ABI provides this service in all 67 Pennsylvania Counites.
