Services Offered through A Bridge to Independence

​A Bridge to Independence provides Service Coordination to individuals enrolled in various programs and waivers. Some of the other services provided through waiver programs include:

Personal Assistant Services

Personal Assistance Services are in-home services provided to consumers/participants to assist them with remaining independently in the home and community setting.  PAS services include assistance with daily living tasks, such as (but may not be limited to), bathing, dressing, meal preparation, household tasks and health maintenance activities. 

Personal Emergency Response System Unit

Personal Emergency Response System is provided to enable the consumer/participant to secure help in the event of an emergency.  The system is usually connected to the consumer/participant’s phone and is programmed to send a signal to a response center when the “help” button is activated during an emergency situation.  However, various PERS providers now also offer a unit that does not require a phone line to operate, and instead, operates using a cell phone tower signal in the area. 

Non-Medical Transportation Reimbursement

Non-medical transportation services enable the consumer/participant to gain access to waiver services and other community resources and activities.  Non-medical transportation services include, but may not be limited to, costs for driver(s), or reimbursement for the purchase of tickets or tokens for public transportation for the consumer/participant.

Community Integration

Community integration is a goal-based support service designed to provide short-term assistance to consumers/participants with developing, maintaining, and improving self-help, communication, socialization and adaptive skills.  Community integration services include, but may not be limited to, cueing and on-site modeling.

Nursing Home Transition

Nursing Home Transition services are designed to empower consumers/participants, who are currently living in a nursing facility, and who wish to return to a home setting within the community.  Services include but may not be limited to, costs for the expense of establishing basic living arrangements and help with moving into the community.  Nursing Home Transition staff may also help to consumer/participant to locate housing, assist with home modifications, and arrange for in-home care.  ABI can assist with directing you to your local NHT entity upon request.
