What is a service coordinator and what do they do?
A service coordinator works with you and your specific needs to help you find and access services available to you. They help you understand the different services you qualify for and help you choose the best option for you and your individual needs.
What is a supports coordinator and what do they do?
A supports coordinator is a trained staff person who assists an individual with locating services in the home and community setting. A supports coordinator will also monitor options for all supports in your plan, including informal, mainstream and community needs.
