Community Health Choices
In August of 2016, the Departments of Human Services (DHS) and Aging made an announcement of their decision to move forward with the implementation of Community Health Choices (CHC) - PA’s plan for the delivery of both physical health and long-term services through a capitated managed care system. To assist with the implementation of CHC, the following three managed care organizations (MCOs) were selected by DHS:
Community Health Choices was developed with the intent to improve coordination of physical and behavioral health systems, as well as Long Terms Services and Supports (LTSS).
Enrollment into CHC will include individuals 21 or older who:

CHC Program Phases
Individuals with disabilities and senior Pennsylvanians can expect CHC to be rolled out in three (3) phases: Phase 1 Scheduled to start January 1, 2018. This phase will affect the following 14 counties (Southwest PA): Allegheny, Armstrong, Beaver, Bedford, Blair, Butler, Cambria, Fayette, Greene, Indiana, Lawrence, Somerset, Washington and Westmoreland. Phase 2 Scheduled to start January 1, 2019. The rollout will affect the following counties (Southeast PA): Bucks, Chester, Delaware, Montgomery and Philadelphia. Phase 3 Scheduled to start January 1, 2020. This final phase will affect the remaining 48 counties in the state. |
