A Bridge to Independence is proud to announce the opening of a new office in Philadelphia at 1516 N. 5th St., Suite 235. This expansion marks an exciting milestone for our organization and a renewed commitment to serving individuals with disabilities and older adults throughout Southeast Pennsylvania.

For over a decade, A Bridge to Independence has been dedicated to empowering individuals with physical and intellectual disabilities to live as independently as possible. With a team of compassionate, knowledgeable Supports Coordinators across the state, we have helped thousands of individuals navigate complex systems and access the needed services and support. Now, with a physical presence in Philadelphia, we are ready to deepen our impact in a region rich in diversity, culture, and community need.

More Than a New Location—A Stronger Presence

The decision to expand into Philadelphia wasn’t just about growth—it was about purpose. As one of the largest metropolitan areas in Pennsylvania, Philadelphia is home to thousands of individuals eligible for home and community-based services (HCBS). These services, funded through state Medicaid waivers and managed care organizations, can help people with disabilities and older adults remain in their homes, stay connected to their communities, and maintain dignity and independence.

At our new Philadelphia location, A Bridge to Independence will provide the same high-quality Supports Coordination services we’ve become known for statewide. Our local team will be available to support individuals who are aging or living with disabilities—including those with autism, intellectual disabilities, and physical challenges—through personalized case management, resource navigation, and ongoing monitoring.

We know that every person’s needs are unique. Whether someone is seeking help applying for waiver services, finding accessible housing, connecting to health care providers, or exploring opportunities for community engagement, our coordinators are here to walk with them every step of the way.

A Bridge to Independence is an accredited Supports Coordination Organization (SCO), which means we don’t provide direct care—we provide guidance. We work with individuals and families to identify their goals, understand their needs, and then help connect them to the right services through Pennsylvania’s various waiver programs.
