What is the CHC Waiver in PA?

The Community HealthChoices (CHC) Waiver Program is Pennsylvania’s long-term care waiver for the delivery of both physical health and long-term health care services through a per-patient managed care system put in place by the Pennsylvania Department of Human Services (DHS) and Aging. The goal of this long-term care waiver is to improve coordination of physical and behavioral health systems, as well as Long-Term Services and Supports (LTSS).

Any long-term services and supports listed above are offered to participants on top of their physical health benefits package. Individual Managed Care Organizations (MCOs) may choose to exceed the CHC service requirements and offer additional services to their participants.

Those eligible for enrollment into the CHC Waiver are PA residents 21 or older who:

  • Receive both Medicaid and Medicare (i.e., Dual Eligible adults)
  • Receive long-term services and supports in the Attendant Care Waiver, Independence Waiver, CommCare Waiver and Aging Waiver
  • Receive nursing home services paid for by Medicaid
  • Are OBRA Waiver participants who were found to be nursing facility clinically eligible
  • Are Act 150 Program participants who are eligible for both Medicaid and Medicare
